1st DOSE DRIVE-THRU MASS VACCINE CLINIC IN ROLLA, MO Jordan Valley Community Health Center in partnership with Phelps-Maries County Health Department, Phelps He...alth, Missouri S&T, the Missouri National Guard, the Missouri Department of Health and Senior Services, and the Missouri State Emergency Management Agency is holding a mass COVID-19 vaccination event in Rolla, MO for all Missouri residents ages 16+ on Friday, April 9th. o Step 1: Individuals need to register on Missouri’s Vaccine Navigator site: https://covidvaccine.mo.gov/navigator/ or call 877-435-8411 to get an 8-10 digit Patient Id Number. o Step 2: Using that 8-10 digit Patient ID Number, individuals will schedule an appointment using this link: https://bit.ly/3fCZ970 Event Flyer: https://bit.ly/3sNT8Ia Consent Form: https://bit.ly/3sL0hJu
